2017 Awards of Excellence

The OKPCA Awards of Excellence honor those that are working to improve access to healthcare in Oklahoma. The changing of the OKPCA Awards of Excellence format in 2017 allows each organization the opportunity to select and honor individuals, as well as highlight a recent success, within the organization. We invite you to select individuals whose accomplishments best exemplify excellence in health care and contribute to OKPCA's mission of strengthening access to affordable community-based health care. At the discretion of each membership organization, an Employee of the Year and a Board Member of the Year may be awarded during the Awards of Excellence Luncheon at the 2017 Annual Conference. The awardees will be highlighted in the 2017 OKPCA Awards of Excellence digital publication Program, available through OKPCA's Annual Conference App. 


Awards of Excellence Submission Process

All entries must include a statement identifying awardee achievements, short testimony, thank you or acceptance quote, head shot photo and completed organization submission form.

Awardee Submission Form

Please fill out all fields within both the Employee of the Year and Board Member of the Year submission form for each that will be awarded by your organization at the Awards of Excellence Luncheon, October 12th, 11:45 AM - 1:30 PM at the DoubleTree Warren Hotel in Tulsa. The 2017 Awards of Excellence will be selected by each health center at their discretion. Each submission should be the organizations finalist. OKPCA will ot be responsible for evaluating submissions.

Organizational Highlight

Please fill out all fields within the submission form. Within the 2017 Awards of Excellence Program members will highlight a recent success and/or information about the organization. Keeping within the space guidelines, members have the opportunity to showcase services, projects, staff or other positive encounters the organization would like to share with peers, partners, stakeholders, and friends of the health center program.

To ensure consistency in the Award Program, please note the following guidelines:

  •  The organization highlight should be no longer than 731 characters with a picture, or 1333 characters without a picture
  • The organization success should be no longer than 1227 characters with a picture, or 1862 characters without a picture

Awardee Achievement Statement

The statement provided within the submission form will be published in the 2017 Awards of Excellence Program. Include narrative that identifies awardee achievements, how the awardee fulfills OKPCA's mission of strengthening access to affordable community-based care, and their impacts on patietns, the organization and/or the community. In addition to the organizations statement about their selected award winner, a short testimony, thank you, or acceptance quote from awardee is highly suggested. Due to time limitations, very brief presentations will be made and acceptance speeches aren't permitted. 

To ensure consistency in the Award Program, please note the following guidelines:

  • Each achievement statement should be no longer than 910 characters
  • The awardee quote should be no longer than 307 characters

Cost

Due to the significant increase in number of awards the Program Planning Committee decided to pass the cost of the awards on to the member organizations. A new award is being selected, and a sample picture of it will be shared out once available. The cost of each award will be $25 or less. OKPCA will send invoices for the awards after the OKPCA Awards of Excellence Submission deadline.

Award selections must be submitted via email to okpcameetings@okpca.org by 5:00 P.M., Thursday, August 31, 2017.
