DESCRIPTION:\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nLevel Up – CHW Trainings on Diabetes and Hypertension: Join Tobi Bratten, MSN, APRN-CNP with Great Salt Plains Health Center for this two-part training to prepare Community Health Workers (CHWs) and care teams to optimize clinical workflows and improve outcomes for patients with hypertension and diabetes.\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\n\nFocus areas will include:\n\nHome visits\nBlood pressure & blood sugar self-testing\nMedication side effects & adherence\nDisease-specific education\nTroubleshooting problems\nIdentifying & using community resources\nCommunication with providers\nImproving clinic workflows & care team productivity\n\nThese trainings will be held in OKC. The CHW Diabetes training will be held Friday, June 13, and the CHW Hypertension training will be held Friday, July 11. We are aiming to make it available virtually, as well, but that will depend on the event space, so help us finalize logistics by registering ASAP! There is no cost to attend.
