Meadow Hazelhoff

Meadow Hazelhoff, MSW, LCSW – Director of Behavioral Health Services

Meadow Hazelhoff is the Director of Behavioral Health and Special Populations at Oklahoma Primary Care Association, where she provides training, technical assistance, and support to community health centers across the state.

She earned both her Bachelor of Arts and Master of Social Work degrees from the University of Oklahoma, where she was recognized as the Outstanding Senior in Professional Programs (2015), an EPIC Scholar, and the recipient of the Anne and Henry Zarrow Award (2016).

Her professional experience includes serving as an integrated behavioral health consultant in a community health center, working in a specialized therapy setting, and operating a small private practice since 2020. She has extensive expertise in integrated behavioral health models and workforce development.

Meadow has supervised graduate-level social work, counseling psychology, and psychiatric nurse practitioner students, as well as serving as a clinical supervisor for LMSWs. She is also a board-approved clinical supervisor.

In 2022, she was appointed to the State Planning and Advisory Council to the Oklahoma Department of Mental Health and Substance Abuse Services, and she continues to serve on several statewide behavioral health councils, work groups, and collaboratives.

Meadow’s professional mission is to expand access to high-quality, inclusive mental health care for all Oklahomans.
