Value-Based Contracting for
Community-Based Organizations
The Patient Care Network of Oklahoma began in 2014 with the dream to improve the lives of Oklahomans through healthcare. Over the past few years, PCNOK has grown while pursuing improved healthcare, a healthier population, and intelligent spending.
Currently, 23 organizations are represented through PCNOK. To learn more about membership, visit our Member Services page or contact us at [email protected].
The Impact of PCNOK
As the largest network of primary care providers in the state, PCNOK serves people living in all 77 Oklahoma counties. All life cycles, from prenatal care through geriatric care, are part of the overall care model. PCNOK members operate 150+ urban and rural health care delivery sites—most of which are designated Patient Centered Medical Homes offering medical, dental, vision, mental health, and certain specialty care, as well as diagnostic testing, pharmacy, and nutrition with referral to outside specialty care as needed.
PCNOK members work collaboratively to advance innovation in care delivery, including integration of mental health, tele-health, care teams, and health coaches into the scope of primary care.
